Eden HHC is looking for experienced Registered Nurses and Vocational Nurses to join our dedicated team. Our service area includes Los Angeles County, Orange County, Ventura County, San Bernardino County and Riverside County. We use Kinnser/Wellsky for EMR.

Licensed Vocational Nurse

- Visiting Nurse/Per Diem

- Private Duty/Shift Care

Candidates must have:

  • Current active LVN license in the state of California

  • Current CPR, Health clearance, TB clearance

  • Minimum of one year of professional nursing experience, preferably in Home Health

  • Must pass criminal background check

  • Driver’s license

  • Proficiency in EMR charting and timely submission of documents

  • Proficiency in wound care, catheter insertion, tracheostomy care, diabetic care, vital signs monitoring, ostomy care

Responsibilities:

  • Complete on-going comprehensive assessments of the patient’s needs

  • Provide service and /or treatments requiring substantial and specialized nursing skills

  • Provide skilled nursing care under the direction of the RN or the physician

  • Educate and support the patient, family and caregiver in meeting nursing and related needs

  • Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance.

  • Implements the patient's individualized plan of care and participates in case conferences

Candidates must have:

  • Current active RN license in the state of California

  • Current CPR, Health clearance, TB clearance

  • Minimum of one year of professional experience (Home Health Preferred)

  • Must pass criminal background check

  • Driver’s license

  • Proficiency in OASIS charting and timely submission of document

Responsibilities:

  • Complete initial and on-going comprehensive assessments of the patient’s needs

  • Provide service and /or treatments requiring substantial and specialized nursing skills

  • Initiate appropriate preventative and rehabilitative nursing procedures and therapy referrals

  • Educate and support the patient, family and caregiver in meeting nursing and related needs

  • Perform duties consistent with the Nursing Practice Act including the Standards of Competent Performance.

  • Initiates plan of care and participates in case conferences
